Every reversal is personally performed by Dr. Rahman — never delegated — and he is prepared for both types of repair in every case:

- Vasoepididymostomy — a far more complex bypass to the epididymis, required in some longer-interval cases. Few surgeons in the region offer it; Dr. Rahman decides in the operating room based on your anatomy, so you get the right repair the first time.
Timing matters — success is highest when the reversal is performed within a few years of the vasectomy, though pregnancies are achieved even after much longer intervals. After surgery, Dr. Rahman personally follows your semen analyses every couple of months until your counts stabilize or pregnancy is achieved, so nothing is left to chance after you leave the operating room.

How a microsurgical reversal works
Every step is performed personally by Dr. Rahman under a high-powered operating microscope. The entire operation typically takes 2–3 hours and is done as an outpatient — you go home the same day.
Access the vas
Through a small incision, both ends of the vas deferens are gently isolated where the vasectomy was performed.
Prepare & test
The scarred segment is removed and fluid from the testicular side is examined for sperm — this finding determines which repair is right.
Microsurgical repair
Under the operating microscope, the ends are reconnected in layers with sutures finer than an eyelash, aligning the inner channel precisely.
Recovery & follow-up
You go home the same day. Semen analyses every couple of months confirm the return of sperm until counts stabilize or pregnancy is achieved.
Why the microscope matters
The difference between a good reversal and a failed one is measured in tenths of a millimeter. Working under high magnification allows Dr. Rahman to:
- Align the inner channel precisely — the mucosal layer is joined edge-to-edge with sutures finer than a human eyelash, so sperm flow isn't blocked by even a slight offset.

Nadeem Rahman, MD
Dr. Rahman graduated from Duke University School of Medicine and completed his urology residency at the University of California, San Francisco, where he operated with some of the most renowned surgeons in the world. He has co-authored numerous nationally recognized publications, spoken at national and international meetings, and was awarded the Robert Krane Prize for the top research paper in the British Journal of Urology.
After beginning his career in Arizona, Dr. Rahman returned to California to serve the Central Valley, where he has practiced for more than two decades. He is board-certified in urology and affiliated with Saint Agnes Medical Center, Clovis Community Medical Center, and Fresno Surgical Hospital.

Does vasectomy reversal work?
Yes. When performed microsurgically by an experienced surgeon, reversal restores sperm to the ejaculate in the large majority of men. Success depends on the years since your vasectomy, the findings at surgery, and — most of all — the surgeon's technique. Dr. Rahman is prepared for both types of repair in every case, so you get the correct operation the first time.
How long after a vasectomy can it still be reversed?
There is no absolute cutoff. Reversals are routinely successful even 10–15+ years after vasectomy, though the type of repair required may change with time. A consultation is the best way to understand your individual outlook.
What makes one reversal surgeon better than another?
Microsurgical skill. The inner channel of the vas deferens is about 0.3 mm wide — reconnecting it accurately requires an operating microscope, ultra-fine sutures, and a surgeon with dedicated reconstructive training. Ask any surgeon whether they perform the repair under a microscope, whether they can perform a vasoepididymostomy if needed, and whether they personally perform every step.
What is recovery like?
Most men are back to desk work within a few days and resume full activity within a few weeks. Sperm typically returns to the semen within a couple of months (occasionally longer, especially after epididymal bypass). Dr. Rahman follows your semen analyses every couple of months until counts stabilize or pregnancy is achieved — and once sperm returns, fertility can last for many years.
